Disability Plates for Facilities

A facility that owns a vehicle used to transport disabled patients, such as a nursing home, is eligible to receive disability plates for the vehicle. 

How does a facility obtain disability license plates?

Prior to receiving disability plates, the facility must have already titled the vehicle. A representative from the facility then can complete and submit the Application For License Plates For Facilities, which is available online, at any branch office, or by calling the Customer Service Center.  If the vehicle is leased, a representative of the leasing company must also sign the form, or you will need power of attorney from the leasing company. The completed form can be submitted in person or by mail to any full-service MVA branch office

Can a facility receive disability placards (hang tags)?

Facilities are not eligible to receive disability placards for their vehicles.


Fees:

  • If a set of disability license plates is requested at the time of the initial registration, the fee is the same as the standard registration plates.
  • If a set of disability license plates is requested after the initial registration is issued the substitute tag fee is charged.
