Request for Motor Vehicle Administration Records (form # DR-057) is an application form used to request motor vehicle records. 1/2 and 3/4 Ton Truck Owner's Weight Certification (form # VR-142A) is used when an owner of a 1/2 or 3/4 ton truck requests to carry a gross vehicle weight of 10,000 pounds (instead of 7,000 lbs). 1/2 ton trucks must be modified to carry the increased weight and 3/4 ton trucks must have a shipping weight of the chassis and body of more than 1,500 pounds or rated by the manufacturer at a gross vehicle weight in excess of 7,000 pounds. Application for Salvage Certificate/Owner Retention (form # VR-028) may be used by an insurance company who acquires a vehicle as a result of a claims settlement, an Automotive Recycler or Dismantler that acquires a salvage vehicle from a source other than an insurance company, a vehicle has been acquired as an abandoned vehicle from a police auction, or by any other person who acquires or retains ownership of a vehicle that is salvaged. Certification for the Issuance of Dump Service License Plates as Provided by the Maryland Vehicle Laws (VR-098) used by customers applying for Dump Truck registration to certify that the vehicle meets all of the requirements under Maryland Law for this type of registration. | |

Low Speed Vehicle Application (form # VR-324) is used when a four-wheeled electric vehicle that has a maximum speed capability that exceeds 20 miles per hour but is less than 25 miles per hour is titled and registered in the State of Maryland. Maryland Dealer Reassignment (form # VR-182) is used by a dealer to assign ownership of a vehicle. | |
Motorcycle Equipment Certification (form #VR-094) is used by customers and dealers to certify that the motorcycle meets requirements for registration and also informs customers of restrictions of motorcycle use depending on the engine size of the vehicle. | |
Odometer Disclosure Statement (form # VR-197) can be used by a license dealer in the State of Maryland. Allows the disclosure of a vehicle's odometer mileage from the transferor (seller) to the transferee (buyer). This form should accompany the title transaction. | |
Schedule of Fees (form # VR-042A) used by dealers, customers and the MVA to determine the correct fees, both full year and half year for larger trucks, tractors, dump trucks, etc. | |
Security Interest Filing Statement (form # VR-217) can be used to record a lien on an existing title certificate or for the original lien holder to reassign the security interest in a vehicle.
